| Civil Awareness and Request Form | Use this form if you wish to inform the Borough of concerns or make a request. |
| Local Service Tax Standard Form | This form should be printed and completed by ALL and ANY Businesses in town. |
| Local Service Tax Exemption Form | Fill out this form if you believe you are exempt from paying the local service tax at your job. |
| New Residence Form | All new residents must complete this form at the time of moving into the Borough of Carrolltown. |
| Pavilion Rental Form (Park & Recreation Form) | Use this form AFTER you verified with the Borough Office the date you chose to rent your pavilion at the park. Send it in with your information and a check to secure the pavilion. |
| Peddler’s Permit Application | Fill out this form when you or a group are going from house to house within the Borough of Carrolltown. Bring it in filled out along with a check allowing you three days within the Borough and expect to have your driver’s license and another form of ID with you plus all information pertaining to the vehicles used, including insurance. |
